Applications
must be accompanied by a signed release in order to
be considered for inclusion in the City of the
Hills.
Release:
I hereby release and
forever discharge The City of the Hills Fine Arts
Festival, all sponsoring organizations and their
directors, officers, employees, agents and
volunteers from any responsibility, personal
liability, claims, loss or damage arising out of or
in conjunction with my participation in City of the
Hills Fine Arts Festival. City of the Hills is
not responsible for any injury sustained by
exhibitors or guests while within space designated
for exhibits. By signing below, I acknowledge
having received and read the 2008 City of the Hills
Fine Arts Festival application brochure and agree to
abide by the rules set forth therein. I also
give The Arc Otsego permission to use my name,
photos and images of my work for publicity purposes. |
To be considered for inclusion in City of the
Hills, all applicable fees must be sent within
10 days of submitting this on-line application.
